It is assumed that one of the custodians seen in "The Misery Chick" is Pavlov, even though this is not specifically mentioned.  If this is the case, then Pavlov is partially responsible for the untimely death of Tommy Sherman, as he neglected to remove the packing material for the new goal post, resulting in Tommy's stumbling over it and being crushed.
